Post School Transition Checklist for Professionals

This checklist is for professionals directly involved with the young person to help ensure appropriate actions are taken in the process of post school transition planning. This document refers to the VINCYP: Moving On and Interagency Advisory Report, links to which are below.

VINCYP Moving on for Young People who have a Visual Impairment and VINCYP – Moving on for Young People Easy Read

This is a project for the young person to undertake in their final years at school , with appropriate support from their parents and teachers, for them to find out about matters which are important to them, and what they need to know about life after school.

Interagency Advisory Report  – to assist professionals when meeting together to identify and provide the appropriate services for each individual child and young person in a coordinated way

Over 16 Years of Age

Young people over the age of 16 years, who meet the criteria, can be registered as sight impaired or severely sight impaired.  Below are some useful links to information that is sent with the letters that the VINCYP Network issues to young people and their families before they transition into adult services:

When choices and decisions are being made it is essential that the young person’s rights and wishes are listened to.  It is the right of every child and young person to be involved in any decisions that directly impact on their lives, including when the time comes to move into Adult Services.  The following are useful links which can provide you with further information about the process and support your transition:

This guideline covers the period, during and after a young person moves from children’s to adult services. It aims to help young people and their carers have a better experience of transition by improving the way it is planned and carried out. It covers both health and social care. This guideline is not specific to Visual Impairment.
