The current post mortem authorisation forms, clinical guidance notes and information leaflets are listed below (November 2006 versions).<\/p>\n\n\n\n

All NHS sources requesting post mortems should have hardcopy stock. Where you don’t have stock please contact your local pathology department who can advise on further supplies<\/strong>. If you have difficulty sourcing forms locally please contact the printers directly by emailing alan.mcguire@theapsgroup.com<\/a><\/p>\n\n\n\n

The hard copies should be used in preference to the electronic copy listed here. In exceptional circumstances, where the hard copy is not available<\/strong> the electronic copy can be used. In this situation the form should be completed and signed as appropriate and photocopies taken for the pathology department and the patient’s notes. The original should be given to the relatives authorising the post mortem<\/strong>.<\/p>\n\n\n\n
